Subject access request

As a customer, you have the right to see and get a copy of your personal data and other additional information. This is called a subject access request or ‘SAR’. You can make SARs verbally or in writing.

Once you make a SAR, we’ll contact you within a month with the information you’ve asked for if it’s held on our systems and no exemptions apply.

Sometimes, we allow a third party, such as a solicitor or someone with power of attorney, to make a SAR on behalf of another person. To do this, we’d need written permission from the data subject/person who has appointed the third party. There may also be situations where we need extra time to respond to a SAR, or in some cases, we may need to refuse access to personal information. We can also refuse to provide the information if the request is unfounded or excessive.

You can find more details about these situations in sections four and five of our Data Protection Policy

To make a SAR, please complete the following form. You can also contact us via dpo@chp.org.uk to make a request.

For further information or help with making a SAR, please email dpo@chp.org.uk.
